it slips over the eshaft bolt, and then uses 2 of the 4 bolts on the crank pulley to mechanically keep the eshaft bolt in place.
dont know the price, but its a little peice of stamped steel, cant be more than $50

yeah... i think they came on some of the FCs... i used to have a few lying around the garage. Exactly what is the question here?

the only one i've ever seen come loose was due to an end-play issue. that engine didn't stand a chance no matter what you bolted to the front of it.
