Reading The "hold" Codes....

Reading through the service manual, and searching on this site, I couldnt find an easier way to read the "hold" codes for the auto trans. It seems like it works very similar to how you check the ECU codes, by just using a jumper pin, but what are the specific "jumper" points to pull the hold codes?
