I've heard than on older rotors thepins can break and the gear can come loose. Not very common though...

on all rotaries the pins can unroll a little and force the rotor gear outward if rotor/stat gear loads are to high (to many rpm on to heavy of rotors), the load on the gears doubles from 8000 to 9000 rpm and keeps getting worse and worse
