Driving home, something in the clutch made noise, like the adjuster rod came loose again, and I was able to get it to the side of the road...adjuster rod was fine, albeit at the end of the engagement...I turned it back a few twists, and now with the clutch out the pedal vibrates...WTF?

How old's the clutch? could you possibly have bent the clutch disk a little, or worn out a bearing?

you know... mine used to do the same thing, but when i pushed the pedal in just a bit. I just changed out the whole clutch since I was slipping anyways. Maybe a throw out bearing? I know when I replaced the clutch it was making a loud ding-ding noise very rapidly, like someone ringing a bell. When I replaced the throw out bearing it stopped. That's my guess.

I don't know if its just the NA s4's but I swear every one I've had has noise from the TO bearing even after only 5k miles of replacing it. On the 86 I had the trans rebuilt and a new Mazda stock clutch & bearings put in and it ended up making noise around 10k miles.
